DMHS recently launched an innovative program to deliver health services to seniors at home, in partnership with the Durham Non-Profit Housing Corporation. Housing Corporation residents 55 and older can receive a combination of scheduled and on-call support from a team of Personal Support Workers, a Recreational Therapist and a Registered Nurse. Currently, three Housing Corporation residences are supported: Wood Farm Manor (1525 Nichol Avenue, Whitby), Perry Terrace (220 Perry Avenue, Whitby), and Cormack Station (250 Hickory Street South, Whitby).
The program, which will improve quality-of-life and reduce unnecessary hospitalizations and emergency room visits, was featured in a recent article in Metroland papers.
